After your entry form has been received, you will be sent the name
and address of the coordinator of the category in which the title has
been entered.
Read the Rules first, then print out the Entry Form.
Deadlines:
Deadline for receipt of entries and checks: November 20, 2004.
The deadline for receipt of books: January 10, 2005.
